Wayne County Four Star uses a 3-share model in which the monthly premium cost is shared by the employer, the employee, and Four Star, each paying $62 per month.

Thank you for your interest in the Wayne County Four Star Health Program. We’re excited to have you learn more about this important program. Once you’ve had a chance to review the section for agents and look over the benefits, I will be available to answer any questions you may have, and help you become a designated Four Star Representative. It’s a very easy process.
Essentially, Four Star is a three share program. The premium is 186.00 divided three ways between the employer, the employee and a non-profit fund – in other words, 62.00 per party. The carrier is HAP, a name we all know and trust. And the participating health systems are well known, too. They are St. John Providence Health System, Henry Ford, DMC and Oakwood. So, this program is quite easy to sell. Your customers will be very happy with the value they’ll be receiving at a price that you will not find anywhere else.
