Four Star Health is a community based not-for-profit organization developed to provide affordable health coverage.

Four Star is an employer sponsored program for low and moderate income uninsured employees in Wayne County.

Wayne County Four Star is a 3-share model in which the premium cost is shared between the employer, the employee, and the county, each paying $62.00 per month.

YOUR BUSINESS MUST BE PRIMARILY LOCATED IN WAYNE COUNTY

YOU MUST BE IN BUSINESS AT LEAST SIX MONTHS

MUST NOT HAVE OFFERED A GROUP HEALTH PLAN TO EMPLOYEES IN
THE LAST 12 MONTHS (SEE IMPORTANT NOTE BELOW)

YOUR BUSINESS MUST HAVE 2-100 EMPLOYEES

YOU MUST HAVE 2 EMPLOYEES ON PAYROLL WHO WOULD
ACCEPT COVERAGE (I.E., NO 1099 CONTRACT EMPLOYEES)

AT LEAST HALF OF ENROLLING EMPLOYEES MUST LIVE IN
WAYNE COUNTY

AT LEAST HALF OF EMPLOYEES MUST MAKE $16.00 AN HOUR OR LESS

YOU MUST HAVE AN ACTIVE “WORKERS COMP.” POLICY

Important Note: If your business has offered a group health plan in the last 12 months, but has a low participation in that plan, or has experienced large rate increases in the past two years you may still be eligible.
